Why this matters
As part of the NHS Left Shift agenda, moving care from hospital to community and home will reduce healthcare costs, increase efficiencies, and enable prevention. To achieve this, we need to strengthen primary care and community services, and empower patients to manage their health proactively.
This Living Lab facilitates the co-design and testing of Digital health technologies (DHTs) and self-monitoring tools to support patients in managing their conditions, avoiding unnecessary hospital admissions, and initiating early intervention.
